Is this problem only related to Hybrid Pacificas? Gas only Pacificas don't have these symptoms?
I have a 2023 2WD Pacifica and it also has the loose axle nut noise. So it's not confined to only hybrids.
I have both a video and an audio recording of the clicking noise that's about 30 seconds long, but I can't figure out how to post it here so others know what it sounds like.

To pile on, I believe this is all Pacificas as it was originally reported on AWD versions for which their is a TSB (somewhere)

So, my 2016 built 2017 could also have this problem or only later models?
That I am unsure of. But if you here a clicking or clunk noise from the front end of the van when stepping on the gas then it could be. It can be checked easily by removing the center cap and checking if the axle nut is easily tightened. Mine was very easy to tighten on the passenger side and as soon as I tightened it down the noise disappeared. Mine used a 34mm socket for the nut.

Headphones or ear buds will make it easier to hear the clicking noise from the loose axle nut. The click happens each time the gas pedal is slightly pressed or let off.

So I checked both my front axle nuts this morning and was very happy to find they both were in place, and torqued correctly.
Just in case, I verified and re-torqued them either way to the specified FSM specs: 120 ft/lb.
Btw, my car’s axle nuts use a 32mm socket, not 34mm as others have mentioned on theirs. Mine is a 2019, so I’m not sure if Chrysler went with bigger axles on the more modern models.
The axle threads were a bit rusted since this car was in NY before, rust seems to have helped a bit in this case, preventing the nuts from backing out.. lol
So it makes me wonder now if the clicking noise/feel on the gas pedal is indeed due to this issue of the axle nuts backing out.

is this a hybrid? I had this issue on my hybrid until I got a new transmission.
 
Mine is a hybrid, and because of this, I had to have both sides retorqued this last Friday. 38,000 miles. All is quiet again. There is no more clicking on or off the accelerator at low speeds. (It had the same sound as ICE in the video.)
 
I definitely have this klunk/click noise when lightly applying or letting off the gas at speeds around 30mph.
This is with my 2021 gas AWD Pacifica.

Question: does anyone know if this is dangerous (it's risky to drive, or it could cause worse issues), so address it immediately, or can it wait for the next oil change?

Get it done now or do it yourself if you don't have time for the dealership. I believe the loss of motive power reported elsewhere with the axles breaking completely off may be the result of these loose nuts...

 
I noticed it a couple of months ago and had no idea what it was, but these postings clarified it.
I'll get it into the dealer as soon as they'll take me (2 weeks) - which is only a couple hundred miles from now (I don't drive much, and I'll take my chances).

Our 2023 AWD started making the clicking/clunking noise during accel/decel at only 11k miles. After doing some research I checked the axle nuts and sure enough they were both loose. Passenger side was worse than driver side. We happened to be on a road trip a few hundred miles from home at the time I really began to notice the noise, so had to rent a torque wrench and 32mm socket from a local auto parts store to check it. Tightened to 120 ft-lb and noise immediately stopped. Unfortunately, within about 100 miles they had loosened again (passenger side was worst offender as before).

I’ve run across similar issues in the past (GM had a problem with C6 Corvettes and loosening axle nuts some years ago and they went through several TSBs – over-torquing, Loctite, etc.) and one solution that has worked well was using a Nord-Lock washer under the nut. Once we got back home, I ordered and installed the Nord-Locks, torqued to 120 ft-lb and did periodic checks. Like lug nuts, there is an initial re-torque required (after about 50 miles or so). I’m now at over 500 miles driven with the Nord-Locks doing periodic checks and the nuts have not budged after that initial re-torque. I’m confident that this has solved my problem and the center caps are back on the wheels :).

For anyone interested, I ordered directly from Nord-Lock, but you can also get them from Fastenal and probably other places. The correct p/n is NL22sp, which is their enlarged OD (must use the “sp” suffix enlarged OD version under this flanged axle nut), steel design for M22 shaft diameter (washer ID is listed at 23.4mm, which I felt provided a sufficiently tight tolerance while installing) and comes in a pair. It adds thickness to the stack, but you will still have plenty of threads showing after tightening the nuts.

Anyway, I hope this info is helpful to someone.

I'll be ordering two sets just in case for mine and will be bringing my torque wrench and the 18.5mm and 32mm sockets along in long trips.

Bolt size: M22/ 7/8"
inner diameter: 0.920"

Edit: Just placed my order for two sets, comes out to around $50 after tax and delivery charge.

Great to hear. I looked them up on Fastenal website as well but they seemed more difficult to find. Better to just get them from the source.

Thanks again for that information. This would avoid a lot of folks a big headache.
Btw, can the washers be reused after being torqued, if say for example an axle needs replacement?
Honestly, all these American cars that don’t have staked axle washers, should bring these Nord-Locks from factory.
