Intermittent ACC/FCW Sensor Failure

I have the Pacifica PHEV Limited package, and starting about 6 weeks after I got my van I started getting intermittent warnings on the display behind the steering wheel saying that the ACC/FCW sensor was dirty and needed cleaning, which then disables ACC/FCW functionality. Prior to this ACC worked beautifully, and FCW worked as expected at least once that I saw.

I've cleaned the sensor several times after getting this warning, and it's never dirty, but it also happens then clears multiple times sometimes while I'm driving. Worse, when the sensors are working, I've had the "Brake!" warning show on the display behind the wheel, an audible very loud "Brake!" occur and the brakes would automatically engage. This has happened when driving up a hill with no traffic in front of the vehicle while driving 20 mph, in a parking garage while driving 3 mph, in traffic with no vehicles in front of me almost causing a vehicle behind me to rear-end me and several more times.

My dealer said there are no error codes and couldn't proactively replace the sensors unless they could prove something was wrong with them, but they've agreed to replace them if I can provide video of this behavior happening, since in their limited driving of my vehicle they did not observe it. In reading through the forums and the NHTSA complaints, this is an issue also on the gas model Pacifica as well as some Jeep Cherokees. This is really dangerous, and the guidance I was given by my dealer is to disable ACC and FCW, which defeats one of the main safety reasons I purchased this vehicle. Is anyone else seeing this behavior?

While I wait for the diode recall fix, I can either experience my vehicle shutting off unexpectedly or slamming on the brakes unintentionally. Awesome.

This exact thing is happening with my 2017 Pacifica and on top of that, there was no alert when my husband nearly rear ended the car in front of us on the interstate. He had to swerve into the other lane to avoid a collision, but not a peep from the car. But drive down an empty road at 20-40mph and it alarms and a big red "BRAKE!" appears on the dash. It keeps telling me my sensor is dirty even when it isn't. My dealership will not help me and seems pleased to let me know it. I even showed them this thread that this is a known issue. They truly do not care and have no empathy or concern for their customers. I will never by another Chrysler.
