We had this one recently as well on our 2017 Pacifica Hybrid, ~3 months after our high-voltage battery failed and was replaced under warranty. It came along with P2BE0, which was the result of a bubble in the coolant line which had not been properly bled under vacuum during the battery replacement.

Service guy wasn't sure if the P1A21 was a remnant from the battery pack failure or something new, since unlike the P2BE0, it was stored and not active. (Second question-why is there no timestamp with the code??).
