Summary (confidence builder):

1. Not all Pacifica engines fail. Many have provided wonderful service for well over 100,000 miles (2017s started rolling off the assembly line in March of 2016). The Pentastar 3.6L V-6 has been around since 2011. There are millions of them, and they truly are remarkable.
2. Read the maintenance section of your owner's manual.
3. Check oil and coolant levels at least once a month (per owner's manual).
4. Use MS-6395 certified 0W-20 motor oil (Walmart has affordable Quaker State and Pennzoil 5-quart jugs; check back of jug for "MS-6395").
5. To be sure, use genuine Mopar OAT coolant. Never mix with non-OAT coolant.
6. Never let anyone tighten the oil filter cap beyond 25 nm (18 pound-feet). Some tighten to only 13.5 nm (10 pound-feet).
7. Make sure the o-ring that comes with the new oil filter is put in the correct place on the oil filter cap.
8. If check engine light is flashing, pull over in a safe area, turn off engine, and call for a tow.

Details:
Given the number of Pentastar engines built and the number of Pacificas sold, the percentage of failures is fairly small. That isn't much comfort to those listed below who have paid thousands in major engine repairs and replacements.

There are reports by owners herein (and in the NHTSA ODI public database) of being told that their third party extended warranties were denied because they drove their Pacificas in an overheated condition when they should have stopped and had the vehicle towed to a repair facility. They say the warning lights and/or messages that are supposed to appear on the driver's dashboard do not appear. They claim they received no warning whatsoever. It is difficult for this member to prove these claims one way or the other. To date this does not appear to be an issue for those who carry the Mopar extended warranty.

It is hoped Chrysler has identified the issue and is taking steps to prevent similar failures in the future. In the meantime, there are steps you can take to help keep your Pacifica's engine happy and healthy while keeping your fingers crossed that you, too, will not be a victim:

    • * FOR THE BEST OWNERSHIP EXPERIENCE, READ YOUR OWNERS MANUAL. Pay particular attention to the maintenance sections.

    • * CHECK THE LEVEL OF YOUR PACIFICA'S ENGINE OIL OFTEN! The owners manual says to check at least once a month and to use 0W-20 synthetic oil. Be sure the vehicle is level. On my Pacifica I have to wait at least an hour after turning off the engine so the oil drains down from the engine to the oil pan before I pull out the dipstick.
    • * "Under no circumstances should oil change intervals exceed 10,000 miles (16,000km), 12 months or 350 hours of engine time, whichever comes first" per the owner's manual. (Note: the 350 hours guideline is generally a concern for fleet vehicles).
    • * Increase the frequency of oil changes if you drive mainly short trips, or if you tow with your Pacifica, or if you drive in extreme hot or cold environments. If you drive in dusty off road environments or if you leave your engine idling for long periods, the Chrysler folks who wrote the manual consider that to be "Severe Duty" and recommend you change your oil every 4,000 miles. Check your manual!
    • * Chrysler recommends using motor oil with "MS-6395" certification compatibility. The list of compatible certifications is usually written on the bottle. Check these oils: Pennzoil Ultra Platinum, Pennzoil Plantinum, Mobile Super Synthetic, Valvoline Full Synthetic Advanced, Quaker State Ultimate Durability, Mobile 1, Castrol.
    • * It is common for the oil cooler in Pentastar engines to fail. It also houses the oil filter and is located in the top valley of the V-6 where it can get pretty hot. The housing is made of hard plastic that can be cracked or distorted if worked on while it is hot. If changing your own oil, let the engine cool thoroughly and be very careful that you do not tighten the oil filter cap beyond 25 nm (18.4 pound-feet) of force per the stamp on top of the oil filter cap. Overtightening can lead to cracks in the plastic housing or twisting it such that the seals become distorted, both of which can result in leaking coolant or engine oil. The cap requires using a 15/16" (24mm) socket which commonly fits 1/2" rachets. With the 1/2" rachet and an extension for the 1/2" rachet, you have a lot of torque in your hand, so it is very easy to overtighten. (I actually use a 3/8" to 1/2" adapter so I can use the less powerful 3/8" rachet and extension, and tighten mine to only about 10 pound-feet until the top of the flange on the cap is even with the top of the housing; anything over that is not needed. I also painted a white mark at the same 3 o'clock position on both the cap and the housing and stop tightening when the dots match and the flange on the cap is at the top of the housing).
    • * Not only can the act of overtightening the oil filter cap damage the oil cooler, but the force needed to loosen an oil filter cap that was overtightened can damage it even more.
    • * If you have a shop change your oil, have them write on the service order that the oil filter cap should not be tightened above the noted torque specs. Be insistent. They will most likely not let the engine cool down all of the way, but if they follow your admonition about overtightening, there's a good chance they won't damage the oil cooler.
    • * Make sure the O-ring that comes with the new filter is put in the correct place on the oil filter cap. If not in the correct "groove," oil can leak out and it could cause one to overtighten the cap when trying to overcome the resistance of the incorrectly placed O-ring.

    • * CHECK THE LEVEL OF YOUR PACIFICA'S ENGINE COOLANT OFTEN! The owner's manual says to check at least once a month. TO BE SURE, TOP OFF WITH MOPAR OAT COOLANT (part number is on top of your coolant reservoir). This is like checking the canary in the coal mine. If you catch the issue of falling coolant level soon enough, you can avoid some very expensive repair bills. If you let the level fall too low, air will get into the system which can lead to a blown gasket.
    • * Per the owner's manual, "Mixing of engine coolant (antifreeze) other than specified Organic Additive Technology (OAT) engine coolant (antifreeze), may result in engine damage and may decrease corrosion protection. Organic Additive Technology (OAT) engine coolant is different and should not be mixed with Hybrid Organic Additive Technology (HOAT) engine coolant (antifreeze) or any 'globally compatible' coolant (antifreeze). If a non-OAT engine coolant (antifreeze) is introduced into the cooling system in an emergency, the cooling system will need to be drained, flushed, and refilled with fresh OAT coolant (conforming to MS.90032), by an authorized dealer as soon as possible."

    • * To see if your oil cooler is leaking oil and/or coolant, shine a light into the tight and hard to see area below the hard plastic pipe holding the screw-on cap to the oil filter (top passenger side of engine). If you can see the light reflecting back off of fluid, then the oil cooler might be leaking. CHECK THE LEVEL OF YOUR PACIFICA'S ENGINE COOLANT and OIL and top off if necessary as you get your vehicle in for service . It is possible the oil cooler is failing and needs replacing, but if you have managed your fluid levels correctly, you might get by with only having to replace that part. If, however, your coolant has fallen dangerously low, there is a chance your engine will overheat and require more substantial repairs or even replacement.
    • * If the Check Engine/Malfunction Indicator Light is FLASHING, "severe catalytic converter damage and power loss will occur. Immediate service is required." (quote from owner's manual). In other words, STOP DRIVING NOW and get your Pacifica towed to an authorized service center to prevent serious and costly engine damage.

Hi Everyone.

On January 19, 2022 I created a post entitled "Let's talk about those few engine failures in Pacificas." On October 26, 2022 I edited the title of the post to remove the word "few."

What follows is a list of users on just this forum who have reported failed head gaskets leading to loss of coolant and, in some cases, the necessary replacement of engines. As of this original posting, the count is 57 423. I believe this is just the tip of the iceberg. There are similar horror stories in different groups on Facebook and Reddit. Most failures occurred after the 5-year/60,000 Chrysler powertrain warranty expired, often just after the warranty expired.
There is more information after the list, including links to subforums and videos, so please keep scrolling.

If you happen to come across this post and see your user name on the above list, please file a complaint with the National Highway Traffic Safety Administration (NHTSA) if you have not already done so by clicking here. An engine that overheats and becomes inoperable is indeed a safety defect, especially in a vehicle advertised for families with kids.

Hopefully your report, along with the numerous reports of others, will induce the agency to investigate this issue thoroughly and put their pressure on Chrysler to make affected owners whole.

@ChryslerCares, feel free to pass this to upper management with the note, "It appears this issue is getting worse by the day and is not going to go away unless we do something meaningful." It would be good of Chrysler to cover the cost to repair the faulty unit, compensate those that already had the repairs done on their own dime, and issue an extended warranty to all owners for the faulty unit (parts and labor). [Please see EDIT (10/12/2023) just below].

EDIT (10/12/2023): In the year that I have been following this issue, I have become more open to the idea that these engine failures might be caused by owners neglecting to read and/or follow their Chrysler Pacifica owner's manual, and by overzealous technicians. But Chrysler could do a few things to correct the problem:
1. Change the design of the oil cooler adapter so that overtightening the oil filter cap will not cause the adapter to crack and lose coolant and/or oil.
2. Failing that, have the torque setting that is currently stamped on the oil filter cap more pronounced for owners who change their own oil and for professional oil change technicians. It is currently very hard to read or easy to miss entirely. And place a prominent note in the owner's manual that the oil filter cap should be tightened to the correct torque setting or serious engine damage may occur.
3. Somehow mark on the engine coolant reservoir itself a note to check the level at least monthly and to use only Mopar approved OAT coolant in order to prevent serious engine damage.
4. Add a "Low Coolant" sensor to the coolant reservoir with a corresponding warning light and message on the instrument panel.

Chryslers Problem With the Pentastar V6 Oil Filter Housing and Cooler Assembly and What It Means for Oil Filter Assemblers

Interesting videos:
  • from "Online Mechanic Tips" posted in or around June, 2023:
This link to a video on YouTube says there has been a manufacturer update to the infamous oil cooler in Pentastar V-6 engines on 2022 and beyond model years. The new part is backward compatible to pre-2022 models when using the correct O-ring included in the package (must carefully read the instructions). This new part could be part of a campaign by Chrysler to replace failed units with new updated ones while extending the engine warranty for all who own pre-2022 Pentastar engines.

And this link to a video on YouTube shows JT removing the OEM oil cooler from his own car and replacing only the seals which he said were smashed completely flat. In this case he found no cracks in the original oil cooler, just bad seals.

And... this link to a short video on YouTube where JT says Mopar has upgraded the oil cooler by making it stronger, and recommends against getting the Dorman.

And this link to a short video on YouTube entitled, "80% of Oil Cooler Failures are from this."

And this link to a video about how warranty claims are inspected. It includes footage showing that if an aluminum head is warped, it almost certainly results in the engine being replaced because the aluminum block to which it is corrected is most probably warped, as well.
  • from "MotorCity Mechanic" posted in or around September, 2022:
This link to a video on YouTube shows the one correct way (along with a couple of incorrect ways) to install the rubber O-ring that comes with the new oil filter.

And this link to a video on YouTube is a discussion of the Dorman aluminum replacement for the OEM plastic oil cooler. Please note that after this video was published, Dorman came out with a model that is complete with the oil cooler and new oil filter cap (Dorman 926-959), whereas the previous Dorman replacement required one to transfer the oil cooler and filter cap from the OEM assembly.
  • from "carPROblems" posted in or around January, 2023:
This link to a video on YouTube entitled "3.6L Pentastar V6 Overview, Common Problems and Reliability" gives a good summary of engine's history and issues to date. In the summary it states that with over 10 million engines produced to date, the engine has above average reliability and, "with proper maintenance," has an approximate lifespan of 250k to 300k miles.

  • from "NAPA Echlin" posted in February 2025:
This link to a video on YouTube entitled, "Oil Filter Housing Kits | NAPA® Echlin® Professional Education Series" (3:05 min) on the features of NAPA's version of the Chrysler oil cooler adapter. The video points out the common failure points of the original factory unit.
  • also from "NAPA Echlin," but posted a year earlier in February 2024:
This link to a video on YouTube entitled, "Pentastar Oil Filter Housing | NAPA® Education Talk" (32:18 min) on the failure points of the Chrysler oil cooler adapter, and how to avoid many of them using proper installation techniques. Includes discussing the common reasons for failure (overtightening is the primary cause, followed by bad seals). Very informative. (Thanks go to SitKneeBend for finding this).

I remain a Pacifica Minivan fan! They truly are marvelous vehicles. However, I can no longer recommend buying a used one unless the seller can show the coolant and oil levels were always within spec, the correct fluids were always used, and the oil filter cap was never overtightened. For the record, neither of our two Pacificas that were purchased new have had a head gasket failure, but then I do oil changes myself, keep receipts and pictures of the work, and check fluid levels at least monthly; you know, like the owners manual says!

You do realize that class action lawsuits can take years and years for remedy , too funny . Composing a list of a handful of names is lacklustre at best. Two million engines produced with a .001% rate of failure isn’t merit for a judge to certify , there’s specifics criteria that must be met or proven for this . Erroneous thread at best, just saying .

OP here. To all who are experiencing this issue, if your user name is not on the list above and you would like it to be, feel free to PM me or post a reply at the bottom of this thread and I will add it. Thanks!

Can people also post their model year? I am curious if any 2019s have been reported yet. Also would like to know if anyone with the towing package has the issue since that has a beefier radiator and may help.

@CANADAHYBRIDGUY you have constantly been negative about this whole thing. Yes this forum represents a fraction of owners but a sizeable percentage of this fraction are having the same issue. Something needs to be done.

As a follow up to my earlier post it looks like Chrysler is starting to realize there is a problem. I just got this email today - pretty strange to list oil change, transmission check (known recall) and spark plugs.

I’ve never had a car manufacturer tell me that they need to check plugs - that’s a big deal on this vehicle with the removal of the whole intake manifold to check 2,4,6. My guess is they are going to start scoping for damage since many of these vehicles will run for a while with the issue already happening - my vehicle was running when those pictures were taken.

Looks very routine. Wouldn't read anything into this.

So if anyone is willing, can we see if this is related to Mexico vs. US engine manufacturing? If you pull up your window sticker, on the bottom right it shows engine country of origin and it will either be MEXICO or UNITED STATES. Just curious if this is specific to a certain plant or not.

To look up your window sticker, just place your VIN after the =(no spaces) in this url: https://www.chrysler.com/hostd/windowsticker/getWindowStickerPdf.do?vin=

Werd, thank you for this. I've been wondering out loud on this forum about the same thing. Was right in front of my nose the entire time!

Per the window stickers, the engines in both of our Pacificas ('18 & '21) were imported from Mexico. So far, so good; but the '18 only has 47k miles.

Wow this is kinds scary. One of the things that we are experiencing with our pacifica is that my husband says frequently he smells anti-freeze. He has also had to add some but he says the amount he has added is very little. I think the last he mentioned it he said he has added less than a gallon over the last year. Thats actually all he has added since we bought it new in 2019, but he says he has never identified where it is leaking but still smells it sometimes. Hope we aren't going to be added to this list.

Pacifica 2018 Model. The engine light started coming on. The car started shaking strongly at start but gets normal after some minutes. Took it to dealership.

Dealership said same problem you are discussing. Coolant was gone. Head gasket gone. The cylinder 5 misfiring. The engine needs to be replaced.

This problem is affecting just too many owners. Request Chrysler to acknowledge that it is a problem. My car has only done 73000 kms and I have taken very very good care of it with regular maintenance and got it fixed only from flagship dealerships.

Seems warranty just expired month ago. I am the second owner who purchased it at 21K km. The dealership told me that they may not fix it under goodwill as I am second owner. I am left with no options.

Please add my name to the list.

2017 Pacifica, bought used with 33k miles on it. Serviced it sooner than suggested for every oil change. At 73k the engine spiked to hot and immediately went back to normal temp. Low coolant. No signs of a leak. Then rough idle with the P303 misfire in #3 cylinder code. Had coil pack and spark plug changed. Kept having to add coolant till we could get it into the dealership. They misdiagnosed it and replaced the reservoir, which wasn't leaking. A few days later we noticed the new reservoir was low. Kept adding coolant every other day till we could get it back into the dealership. They wanted over $1,000 to tear down the motor to find the leak. Our 3rd party warranty wanted evidence. Then they denied our claim stating that the head was warped due to excessive heat. Duh, that was because of the terrible design flaw in the 3.6 motor. We reach out to Chrysler Cares and they denied us since we are 2nd owners. Add us to the list of casualties caused by a terrible design flaw in this motor. We are still fighting and looking at getting a lawyer involved. This vehicle should not have this type of problem when everything has been serviced timely and correctly and we have all the records to show it.

Your issues literally mirror mine but my van is at 75k and single owner. I have kept with with all the service. My issue originally started in Nov 24 and I had a coil pack and spark plug changed because the issues were like yours....rough idle and it was rough shifting. My mechanic said it needed to go to the dealership because something else was wrong. I barely drove it out of fear of making it worse. It took a month to get in with the actual dealership and they said I needed a new radiator and no other issues were present and then I got it home and it immediately threw several codes on 2 different readers. My van NEVER overheated as I was watching it like a hawk! The dealership towed it back the second time because I refused to drive it and they have had it for 5 weeks now...saying $600 to break down the engine and check for damage. This is sickening as it doesn't seem to be anything WE did to the vehicles and there is no help! The third party always try to wiggle out of paying and this is a massive expense. :(

for everyone an easy fix for it.
I added the head gasket sealent through a mechanic and now my car is running like as fresh as a daisy with no engine light.
The mechanic took 300 Canadian dollars.
Just take care that you take out heat lines when you add sealent to coolant because sealent can jam the heat lines.

Note: adding sealent is just a temporary solution. Do not know how much it lasts. But still better then changing engine. If sealent blows up then you need to change engine. But even without it you had to change it anyhow then why not first run car as much as you can with sealant

I've seen many 2019+ for sale locally with 60k+ miles on them which makes sense since people/fleets dump them once they get a few years old and are out of warranty. Just waiting to see if we have any confirmed similar issues. Hoping that there was some re-tooling or anything that would perhaps make this not an issue. I am in the market and want to pull the trigger and will get a CPO warranty regardless, but don't want to buy into an inevitable failure.

I'm not sure it is safe to assume the issue is a manufacturing problem. We just don't know yet. It could very well be a maintenance problem.

I have topped off the coolant in both our Pacificas with small amounts of Mopar MS-12106 to keep the level at MIN or slightly above. I do not believe it can be ignored for 100k miles, at least not in Pacificas. Perhaps there is indeed a tiny leak somewhere. That might be another possible explanation for the engine failures.

Edit:
Along the "tiny leak" theory, we must remember the number of cases where oil coolers have failed due to folks overtightening the oil filter cap. But, again, would not that be an issue across all Stellantis models with Pentastars?

I am back on Pacificaforums after spending 3 weeks trying to work with Chrysler Cares. I originally posted on "2018 Pacifica Engine Failure at 77,000 Miles", but then Chrysler Cares asked me to contact them privately. They dilly-dallied around for 3 weeks before telling me there is nothing else they can do regarding our engine failure at 77,000 miles. I see they have done the same to others--ask them to contact them privately and then ultimately tell them they won't help. So it appears they just want to get us off Pacificaforums, because there are so many of us who have experienced premature engine failure.

One poster on this site seemed to say a 1 in 100 failure is to be expected, but I don't accept that. If the dealer had told me there was a 1 in 100 chance my engine would fail just outside of the warranty, I would never have bought the vehicle to begin with. And the frequency and similarity of these engine failures makes it highly unlikely this is just some random occurrence. This engine has been a problem since 2010, and Chrysler finally had to acknowledge that for the 2010-2013 model years. But they continue to stick their head in the sand for the newer model years, and hope those affected don't band together and force Chrysler to stand behind their product.

Class action litigation is certainly one avenue, but does anybody have thoughts or experience on filing a Small Claims suit for this issue? In our state (Kansas), the process is quite simple and you are not even allowed to utilize an attorney. Just file your claim and see them in court. Chrysler obviously has deep pockets to fight a class action filing, but what if the thousands of us affected by this inundate them with individual but legitimate lawsuits?

The bottom line is there is an obvious flaw in this engine and Chrysler needs to accept responsibility for it, just like they were forced to do earlier. The question is, what can we do to get them to do the right thing???

For you to succeed in small claims court, I believe you would need to prove what the actual defect is and, as far as I can tell at least, that is still a secret known only by a few who aren't sayin'.

As of this original posting, the count is 57 115. There are similar horror stories in different groups on Facebook. Most failures occurred after the 5-year/60,000 Chrysler powertrain warranty expired, often just after the warranty expired.

Hopefully your report, along with the numerous reports of others, will induce the agency to investigate this issue thoroughly and put their pressure on Chrysler to issue a recall or provide an extended warranty to make fixes for free with no deductible or diagnostic fees, and provide reimbursement to all who have had to pay out of pocket for repairs.

@ChryslerCares, feel free to pass this to upper management with the note, "It appears this issue is getting worse by the day and is not going to go away unless we do something meaningful."
